PMD-D NPC App (Blank)

Since I made an NPC app for the past two groups that I was in, I decided, "Why not make a third?". It's highly influenced by the official drawing apps, I just modified it a bit so it could fit in with an NPC's doings and such. Some of the elements are pulled straight out of them, so if you have any questions for this app, look at those first, then ask me. You're free to use it as you wish, but if you do use it I'd like it if you place a redirect link back to here so that others can find and use this. The Download button has the .zip file that holds the app's .png and .psd versions as well as the example image above.

What my individual things mean:
Guild Affiliation: This is where you list whether your group are a part of the guilds (Rogues, Tradesmen, Explorers) or not (No Affiliation). If the group is affiliated with more than one guild, then put "Hybrid Team" instead.
NPC Group Name: This is where you will put the name of your NPC group. It can be something simple like "Bob and Jacob's Family" or something more expressive, like "Zed and Banjo's EXTREME Baking Corp".
Date of Creation: When did this group come together? You can put the date that you filled it out [25 Oct '14], the relative date of when the team actually came together [2 Years Ago], or something goofy [I Can't Remember].
Cameo Availability: When I made my NPC App for PMDu, everyone a LOT of people misinterpreted the "Cameos" and "Stories" boxes. To try to prevent that from happening again, there's a special spot where you can say whether or not you want people to add your teams to their own pieces of work, a la "Open for Cameos" or "Not Open for Cameos".
Wealth Status: Simply put, "how rich are they?" You can put actual monies amounts (150 PKD [PokeDollars]) or just a general class term, like "Rich", "dirt poor", or "comfortably wealthy".
Number of Members: This is very self-explanatory. How many members are in the group? I suggest the number stay under 7.
Number of Cameos: The number of minor roles that the team (whether in full or in parts) appear within a piece of work.
Number of Stories: The number of major roles that the team (whether in full or in parts) appear within a piece of work.
Basic Info: All of it's self-explanatory. The rectangle under name houses the character's Nature/Characteristic and the one under the moves houses the character's Ability. They cannot have more than one ability.
Inventory: There are four slots. When one is in use, put the name of the item below it. When not in use, either put "Empty" below the empty box or remove the text under it altogether.
Weapons: In the top box, put the name of the weapon. Below it, put the weapon type. (Refer to this journal for the categories.) In the large box, draw the weapon. The box underneath it is its primary move. Under that one is its secondary move. I suggest you not stack together moves of the same category (Special with Special, Physical with Physical). Stacking Status moves is ok, though.
Goal/History: Draw the character's ultimate goal (one they either met or are striving for) and write what it is underneath it.
